Complex Care Center FAQ

Upon discharge from the hospital, an appointment has already been made for you at the Complex Care Center.

Here are our most asked questions

Q. Where is the Complex Care Center located?

A. We are located on the 4th floor of the Medical Office Building in Lee Memorial Hospital. Enter at the main entrance and you will be directed to our office. Wheelchair transport is available when requested.

Q. Why have I been referred here?

A. We provide many services, so you may have been referred for one of the following reasons:

  1. Hospital follow up for patients who have no primary care physician (PCP) or who are unable to see their PCP in a timely manner due to distance or lack of appointments

  2. Wound care – we have providers and nurses who have expertise in wound care

  3. Infusion – we can provide IV antibiotic, blood transfusion or other IV medications

Q. What can I expect?

A. If you have been referred for a hospital follow up, you will see a physician or advanced provider (AP). They will follow up on your issues that were addressed in the hospital. In addition, we will help you find a PCP for ongoing follow up.

  • If you have a wound, you will first be seen by a physician or AP who will set up a wound care plan. Depending on the plan, you may be followed by a wound care nurse for subsequent visits with occasional monitoring by the physician or AP.
  • 如果你来抗生素输液,we will follow the plan from your hospital doctor. Some antibiotics are given daily, for several days or weeks, while something like a blood transfusion may be a one-time visit.

Q. Can I use the Complex Care Center as my PCP?

A. Unfortunately, no. Our goal is to make sure that you are seen promptly after your hospital stay and then we will help you establish with a PCP who will provide ongoing care.

Q. Will I see a specialist at the Complex Care Center?

A. We do not have any specialists. However, if a specialist follow-up is recommended at hospital discharge, we can help set those appointments up.
