No matter which stage of survivorship you are in our oncology team will assist you in any questions that arise. Adjusting to new physical and emotional changes, treatment side effects, and family problems is what many cancer survivors say is difficult adapting to. We provide to you a thorough survivorship care plan. Asummary of the plan includes:
- Diagnosis summary - diagnosis date, cancer stage, type, location
- Family history of cancer
- Cancer screenings and treatments
- Regimen, drug dose, cycles, and clinical trial information

Addressing Fear of Recurrence
Feeling fearful is normal, a cancerreoccurrencemay happen to you after treatment ends. Stay with the survivorship care plan your primary care provided and ask any questions to the specialists you may see. Some ways to cope with the fear of cancer recurrence includes:
- Join a support group
- Follow your cancercare survivorshipplan
- Ask questions to your oncologist
- Spend time with family and friends
